1. Services Provided

NuVita Wellness & Aesthetics provides mobile and in-office wellness services, including but not limited to:

• IV hydration therapy

• Vitamin and nutrient infusions

• Intramuscular injections

• Regenerative and wellness injections

• Aesthetic neurotoxin treatments (where applicable)

All services are administered by licensed medical professionals under appropriate medical oversight in accordance with Texas law.

Our services are elective, non-emergency wellness services and are not a substitute for primary care, specialist care, or emergency medical treatment.

If you are experiencing a medical emergency, call 911 immediately.

2. Eligibility

To receive services, you must:

• Be at least 18 years of age (or have legal guardian consent where applicable)

• Complete all required intake forms truthfully and accurately

• Participate in any required Good Faith Exam (GFE) or medical clearance process

NuVita reserves the right to refuse or discontinue service if treatment is deemed medically inappropriate, unsafe, or non-compliant.

3. Medical Disclaimer

NuVita Wellness & Aesthetics does not diagnose, treat, cure, or prevent disease.

No guarantees are made regarding specific outcomes, results, or benefits of IV therapy, injections, or aesthetic treatments.

All services are provided based on:

• Client disclosures

• Clinical assessment

• Professional medical judgment at the time of service

Individual results vary.

4. Medical Discretion

Licensed providers may modify, postpone, or decline treatment if a service is deemed unsafe or inappropriate based on medical evaluation.

All clinical decisions are made in the best interest of client safety and are final.

5. Appointments, Cancellations & No-Shows

By booking an appointment, you agree to our Cancellation & Refund Policy.

NuVita reserves the right to charge cancellation or no-show fees as outlined in our booking policies.

Mobile concierge appointments may be subject to travel fees based on distance from our base location.

6. Payments

Payment is required prior to or at the time of service unless otherwise agreed upon.

We accept approved forms of payment through secure third-party processors. NuVita does not store full payment card information.

Prices are subject to change without notice.

7. Refund Policy

Due to the nature of medical preparation, sterile supplies, and provider time allocation, all sales are final once treatment has begun.

Refund eligibility is governed by our Refund Policy, which is incorporated into these Terms.

8. Client Responsibilities

You agree to:

• Provide accurate and complete health information

• Disclose all medications, supplements, allergies, and medical conditions

• Follow post-treatment instructions provided by your clinician

Failure to provide accurate disclosures may increase risk and releases NuVita Wellness & Aesthetics from liability related to undisclosed conditions.

9. Assumption of Risk

You acknowledge that IV therapy, injections, and aesthetic treatments carry inherent risks, including but not limited to:

• Bruising

• Swelling

• Infection

• Allergic reaction

• Discomfort

• Temporary side effects

By receiving services, you voluntarily assume all associated risks.

10. Limitation of Liability

To the fullest extent permitted by law, NuVita Wellness & Aesthetics shall not be liable for indirect, incidental, or consequential damages arising from the use of our services.

Total liability shall not exceed the amount paid for the service provided.

11. Indemnification

You agree to indemnify and hold harmless NuVita Wellness & Aesthetics, its owner(s), employees, contractors, collaborating medical providers, and affiliates from claims arising from:

• Inaccurate or incomplete health disclosures

• Violation of these Terms

• Misuse of services

12. HIPAA & Privacy

Protected Health Information (PHI) is handled in accordance with HIPAA regulations and applicable Texas privacy laws.

Use of our website is subject to our:
