Who We Are

NuVita Wellness & Aesthetics provides wellness, regenerative, and aesthetic medical services including IV therapy, vitamin injections, wellness injections, neurotoxin treatments, weight management support, and related services through in-clinic and mobile concierge models.

We are required by law to protect the privacy of your Protected Health Information (PHI) and provide you with this Notice.

  • Telehealth / Good Faith Exam providers (such as Qualiphy)

Mobile Concierge Services

Uses That Require Your Written Authorization

We will obtain written permission before:

  • Selling PHI (we do not do this)

  • Sharing psychotherapy notes (if applicable)
